Where do I get official passport forms?
All forms are free at travel.state.gov — use the online Form Filler to type your answers and print. Never pay for blank forms; third-party sites charging for them are scams.
Can I sign my DS-11 at home?
No. DS-11 must be signed in front of an acceptance agent who verifies your identity. Signing early voids the form and you'll fill out a new one at your appointment.
DS-82 or DS-11 — which renewal form?
DS-82 (mail-in) if your passport is undamaged, was issued when you were 16+, within the last 15 years, and in your current name. Otherwise DS-11 in person.
Do passport forms expire?
Forms are updated periodically — always download the current edition from travel.state.gov rather than using a saved copy. The edition date is printed at the bottom of each form.
